Payment

Fackler Family Chiropractic, LLC has a cash payment policy. This means payment is required at the time of treatment. Payment can be made by cash, check, Visa, Master-card, or Discover cards.

Our office fee is $42.00, an additional $42.00 exam fee will be charged for all first-time office visits. Fees for children depend on the age and extent of the problems. If an after-hours call is necessary, at home or at the office, the cost of the visit will be $80.00. There may be an additional fee for a more comprehensive examination in an automobile accident or a personal injury examination because of paperwork and time involved.

If you are having a financial hardship, please talk to us before any services are rendered. Depending on the circumstances, an alternative payment plan could be implemented at little or no cost.

Insurance

We do not accept insurance assignments or worker’s compensation injury claims. Also, we only accept automobile accident claims and personal injury claims if you pay at the time of the visit.

We are not a Bureau of Worker’s Compensation (BWC) provider, and we are sorry we are not on your HMO’s providers list. We feel there are good reasons why we don’t belong to your plan.  Reasons that could dramatically affect your health. Limiting care is profitable for the HMO but it can compromise patient satisfaction and our reputation. Doctors who sign HMO and BWC contracts often discover that they can’t deliver optimum care. Instead, care is rationed. Patients must be dismissed before the body is totally healed.

In addition, HMO’s and BWC require huge amounts of paperwork. It is time-consuming, costly, and takes away from the doctor’s primary mission of helping their patients get well. Therefore, the price of an office visit would be considerably higher.

Our office works on a computerized charge system. You will receive a receipt which contains all the necessary information you will need to submit your claim through our office. Remember, your insurance company is responsible to you, not us. The fee for treatment is an obligation you have with us and is due at the time of treatment. If you are covered by Medicare, payment is required at the time of your visit, but we will submit the charges to Medicare and Medicare will directly reimburse you.
